Enrollment Information for New Students
Welcome to Amador Valley High School!
We look forward to enrolling your student(s) in our school. If you are new to Pleasanton Unified School District and live within the Amador Valley High School boundaries you can complete the enrollment steps outlined below. Please use the PUSD School Locator to verify your address is within the Amador Valley High School boundaries before you get started.
Step 1: Complete the Pre-Enrollment Online Process
Step 2: Complete the Enrollment Form (50750) English version Spanish version
Step 3: Provide the following documents:

- 2 Proofs of Residency - Accepted Documents
- Housing Questionnaire
- Immunization Record - Shots Required for K-12
- Copy of Birth Certificate or Passport
- Transcript or Report Card
- Request for Student Records form - Click here for Student Records Form

Step 5: Complete the Grade-Level Course Selection Form: (select the correct enrollment year)
- 2022-2023 9th Grade Course Selection Form
- 2022-2023 10th Grade Course Selection Form
- 2022-2023 11th Grade Course Selection Form
- 2022-2023 12th Grade Course Selection Form
Step 6: Email All Documents (Steps 1-5) to Anne Straub: [email protected]
Step 7: Schedule an appointment after you complete Step 6. Please forward all documents at least 5 business days in advance of selecting your meeting date/time in order for records to be reviewed. (Student & Parent Must Attend the Virtual Meeting)
